      This contract is effective upon the signature of both parties below, and upon Performer's receipt of a non-refundable $15.00 deposit to hold the date. The balance due shall be paid in cash, check or money order payable to Allyn Van Dusen on or before the day of performance. Payment of either portion may also be made by credit card (indicate card type, number, expiration date, and name and billing address below). Please sign and return this Agreement with the deposit, and a fully signed copy will be returned to you right away.

* If you have a tape of a song you would like sung but no sheet music is available, I will write it out by hand so that it can be performed - a time-consuming process - hence the extra charge.

Even if you have already received a demo CD, you may be interested my CD "Our Love" which is a wonderful resource if you are looking for song ideas for your ceremony. The 21 beautiful wedding songs on the album are presented with a variety of simple accompaniments (some with piano, some with harp, some with string quartet, and some with other solo voices joining me in a duet). The regular price of this CD is $15.00 ($10 cassette) plus $2.50 shipping. I offer a bridal discount of 10% on both the CD and the shipping cost - so the total cost including New York State sales tax for a CD is $17 (regularly $18.90) or $12 for a tape (regularly $13.50).
