This contract
is effective upon confirmation by both parties (Applicant and
Performer), and upon Performer's receipt of a non-refundable deposit
of $15.00. The balance of the fee shall be paid in cash, check
or money order payable to Allyn Van Dusen on or before the day
of performance.
